Time period Form Description

Program Enrollment Completed by student and faculty advisor to record and evaluate program progression. Once completed, turn in to the Office of Academic Affairs (OAA).

Annually, completed during spring/summer advising


Once completed, signed by student and advisor. A hard copy, as well as a digital copy is sent as directed to OAA.

As needed
Needs signature from new advisor and student.
By 2nd year of coursework This is a form created by The Graduate School. This is used in the PhD Program. A two part form, complete Part I.

 

After passing comprehensive
examination
This is a form created by The Graduate School.  This is used in the PhD Program.  A four part form,         complete Part I.

 


Dissertation proposal defense This is a form created by The Graduate School.  This is used by the PhD Program. Complete Part II of the form – all committee members will sign..

Submitted as needed

  • Transfer Credit Recommendation
Committee must approve all transfer credit.
  • Request Leave of Absence
Used to request a formal leave of absence.
  • Application for Readmission
Used to re-enter a graduate program after being away (not enrolled) during a regular semester (spring or fall). Doctoral Program Director must support the application for readmission. Deadlines are:
Fall – July 1
Spring – December 1
Summer Session I – April 1
Summer Session II – June 1

 

  • Request Reinstatement
Request reinstatement to the Graduate School after having been declared academically ineligible to proceed.
  • Request Extension of Time
Request an extension of time for (a) completion of degree OR (b) removal of IN/AB grade
