| Dissertation defense and approval to submit final dissertation |
Doctoral Exam Report Form | Obtain form from student file in OAA. Complete Part III and Part IV. Return for to OAA. |

| Application for Readmission | Used to re-enter a graduate program after being away (not enrolled) during a regular semester (spring or fall).
