"Paper Trail" of Documentation Ph.D. Requirements for Graduation
 
Time period Form Description

Program Enrollment Plan of Study Used by OAA and faculty advisor to monitor student plan of study. Completed during orientation and turned in to the Office of Academic Affairs.


Preregistration
Periods
Plan of Study OAA gives the advisors Plan of Study forms. Update and turn in to the OAA each semester.

Annually, in spring Annual Report This document is to be completed, signed and sent to the Office of Academic Affairs.

As needed Graduate Advisor Change Form Needs signature from new advisor and student.

By 2nd year of
coursework
Report of Doctoral Committee Composition (Sample Only) A two part form, complete Part I.

Near end of
coursework,
during meeting
of committee
Plan of Study Advisor signature indicating that committee has approved Program of Study.

Transfer Credit Recommendation Committee must approve all transfer credit.
Plan of Study for Minor Committee and faculty from minor must approve if student is doing a formal minor. Before or after committee meeting must be approved by minor program and Doctoral program director.

Doctoral Exam Report Form (Sample Only) A three-part form. Complete Part I of the green form.

Oral comprehensive

Doctoral Exam Report Form (Sample Only) Obtain previously used green form from your student file in OAA. Complete Part II.

Dissertation

Report of Approved Dissertation Project (Sample Only) Second part of Report of Doctoral Dissertation Committee Composition (white form). Obtain previously used form from your student file in OAA. Have committee members sign beside their names.

Research Proposal IRB Tracking Form OAA will stamp and make a copy. Stamped original to be turned in to IRB along with dissertation proposal.

Doctoral Exam Report Form (Sample Only) Complete Part II of green form if not completed earlier during serparate oral comprehensive exam.


Semester of
graduation
Application for Graduation (Sample Only) Must be completed in order for student to graduate. However, no penalty if submitted and student does not graduate.

Deadlines are:
August graduation: 2nd Friday in June
December graduation: 2nd Friday in October
May graduation: 2nd Friday in February

Dissertation defense Doctoral Exam Report Form (Sample Only) Obtain previously used form from file in OAA. Complete Part III of green form.

Submitted as needed Request for Leave of Absence (Sample Only) Used to request a formal leave of absence.
Request for Reinstatement (Sample Only) Used to request reinstatement to the Graduate School after having been declared academically ineligible to proceed.

Request for Extension of Time (Sample Only) Used to request an extension of time for (a) completion of degree OR (b) removal of IN/AB grade

Application for Readmission (Sample Only) Used to re-enter a graduate program after being away (not enrolled) during a regular semester (spring or fall). Doctoral Program Director must support the application for readmission.

Deadlines are:
Fall - July 1
Spring - December 1
Summer Session I - April 1
Summer Session II - June 1
